student health insurance

The San Francisco Art Institute provides a student health insurance plan offered through First Health Network. All degree-seeking students who are enrolled in six or more units are automatically enrolled in the plan.Details for the 2011-2012 plan are coming soon. For information on the 2010-2011 plan offered through Kaiser Permanente, please see plan details below.

Waiving Insurance

Students who have comparable insurance may waive this coverage by completing a Student Accident & Health Insurance Plan Waiver Form. Waiver forms and Proof of Coverage are due by September 12, 2011 for the fall semester and January 31, 2012 for spring/summer.

Coverage

For 2010-2011 (offered through Kaiser Permanente): Fall coverage runs from 8/15/10 through 1/14/11. Spring/Summer coverage runs from 1/15/11 through 8/14/11. Students enrolled at SFAI in the fall and spring semesters will be covered under this plan until 8/14/11. Incoming Low-Residency Graduate students are only covered under the Summer plan, between 5/15/11 and 8/14/11, at a cost of $369.

For 2011-2012 (offered through First Health Network): Fall coverage runs from 8/15/11. Spring/Summer coverage runs from 1/15/12 through 8/15/12. Students enrolled at SFAI in the fall and spring semesters will be covered under this plan until 8/14/12. Incoming Low-Residency Graduate students for Summer 2012 are only covered under the summer plan, between 5/15/12 and 8/15/12.
