Apply for Accomodations
Please complete paperwork and submit it to the Accessibility Services Office no later than January 7, 2013 for accommodations to be in place by the beginning of the spring semester.
These forms must be completed and returned to the Accessibility Services Office in order to begin the process of receiving accommodations:
1. Student Registration Form: Prior to the start of each semester, this form must be completed by the student and submitted to the Accessibility Services Office.
2. Medical Verification Form: This form must be prepared by an appropriately licensed professional that clearly diagnoses a condition, and/or the professional must provide records showing the history of the condition. SFAI requires documentation be prepared within the past three (3) years, or more recently if the specific circumstance warrants it.
About the Accessibility Services Office
The Accessibility Services Office (ASO) ensures that students with documented disabilities have equal opportunity. Our policies and procedures facilitate self-awareness, self-determination, and self-advocacy. We recognize disability and learning differences as important aspects of diversity and work to provide reasonable accommodations to students with documented disabilities. Some common accommodations include extra time for assignments, a quiet place for exams, and note-takers.
If students are unsure whether they will need accommodations or not, we advise that the student complete the process of registering with the ASO so that they will be eligible for accommodations, should they need them.
